Subject Access Request Form

You have the right to request a copy of the information that we hold about you. To request a copy of some or all of your personal information, please complete one of the three options below:

• complete the form below,
• send an email to privacy@ppn.co.uk or
• send a letter to the Data Controller, PPN UK Head Office, Mancor House, Bolsover Street, Hucknall, Nottingham, NG15 7TZ, UK.

We want to ensure that your personal information is accurate and up to date. If any of the information that you have provided to PPN UK changes, for example if you change your email address, contact address or name, please let us know the correct details.
The organisation cannot accept requests regarding your personal data from anyone else, including family members. We may need to contact you for further identifying information before responding to your request. You warrant that you are the individual named and will fully indemnify us for all losses, cost and expenses if you are not. Please allow 28 days for a reply.
